What Are the Treatment Options for TMJ Disorder?

TMJ disorder treatment depends on the underlying cause and severity of symptoms. Options range from at-home care and night guards to neuromodulator therapy and physical therapy. At SPOCS Orthodontics in St. Petersburg, FL, most patients begin with a comprehensive evaluation to identify whether muscle overactivation, bite issues, joint dysfunction, or a combination of factors is driving their pain.

Why TMJ Treatment Is Not One-Size-Fits-All

TMJ disorder has many causes — grinding, clenching, bite misalignment, disc displacement, stress, arthritis — and the most effective treatment targets the specific cause in each patient. This is why a thorough evaluation comes before any recommendation. What works well for a patient whose TMJ pain is driven by nighttime grinding may not be appropriate for someone with disc displacement or joint arthritis.

At SPOCS Orthodontics, our clinical team evaluates each patient individually and presents a clear, prioritized treatment plan. We do not recommend procedures patients do not need.

What Treatment Options Are Available for TMJ Disorder?

The following options represent the range of TMJ treatments in current clinical practice. SPOCS Orthodontics specializes in neuromodulator therapy (marked below) and provides referrals for treatments outside our scope:

Neuromodulator Therapy

Injections of a neurotoxin into the masseter, temporalis, and surrounding jaw muscles to reduce clenching force, relieve pain, and allow the joint to recover. Results begin within 5–10 days and last 3–6 months. Highly effective for bruxism-driven TMJ disorder.

Occlusal Night Guard

A custom-fitted oral appliance worn during sleep to prevent tooth contact and reduce grinding forces on the joint. Protects teeth and reduces joint loading. Often used alongside neuromodulator therapy.

Physical Therapy

Targeted jaw exercises, manual therapy, and postural correction to strengthen and retrain the muscles controlling the jaw. Especially helpful when restricted range of motion or muscle imbalance is present.

Orthodontic Treatment

When bite misalignment is a primary driver of TMJ dysfunction, correcting the bite through braces or aligners can reduce uneven joint loading. Not appropriate for all TMJ patients — evaluation required.

Medication

NSAIDs, muscle relaxants, or low-dose tricyclics may be used short-term to manage acute pain or break a cycle of muscle spasm. Not a long-term solution on their own.

Arthrocentesis / Surgery

Joint lavage (arthrocentesis) or open-joint surgery is reserved for severe cases involving disc displacement with locking, significant joint degeneration, or cases unresponsive to all conservative treatments.

What Does the Treatment Process Look Like at SPOCS?

For patients pursuing neuromodulator therapy at SPOCS Orthodontics, the process is straightforward:

01

New Patient TMJ Evaluation (50 minutes)

Laura Reeger, APRN, FNP-BC reviews your symptoms, medical history, and jaw function. Dr. Ahmed Mansour, DDS is available for orthodontic assessment if bite correction may be relevant. No imaging required at this stage.

02

Treatment Plan Presented

If neuromodulator therapy is appropriate, your clinical team explains the procedure, the products available (Botox, Xeomin, Dysport), expected timeline, maintenance schedule, and pricing. All fees are discussed before any commitment.

03

Neuromodulator Procedure (30 minutes)

Injections are administered at the targeted muscle sites. The procedure is well-tolerated by most patients. Numbing cream is applied beforehand. You leave the office the same day and return to normal activities immediately.

04

Results Window (Days 5–14)

Neurotoxin takes 5–10 days to reach full effect. Patients typically notice reduced jaw tension and fewer morning headaches within the first two weeks.

05

Follow-Up Visit (15 minutes, no charge)

We confirm results and assess whether any adjustments are needed. Maintenance treatments are typically scheduled every 3–6 months depending on your response.

Frequently Asked Questions

What is the most effective treatment for TMJ disorder?

The most effective treatment depends on the cause. For patients whose TMJ disorder is driven by bruxism or muscle overactivation, neuromodulator therapy (Botox, Xeomin, or Dysport) injected into the jaw muscles has strong clinical support. It reduces clenching force, relieves pain, and allows the joint to recover. A night guard is often used alongside it.

Mild TMJ discomfort sometimes resolves with rest and stress reduction. However, chronic TMJ disorder — particularly when driven by habitual grinding or clenching — does not resolve without treatment. Left untreated, it typically worsens and causes additional damage to the teeth and joint.

Timeline varies by treatment type. Neuromodulator therapy produces results within 5–14 days and maintenance treatments are needed every 3–6 months. Night guard therapy may take several weeks to show meaningful improvement. A comprehensive treatment plan with clear milestones is provided at your initial evaluation.

Although this program is cash-pay and services are not submitted to insurance, SPOCS Orthodontics will provide itemized receipts and, upon request, assist in preparing documentation (such as superbills or letters of medical necessity) that patients may submit to their insurance carrier for potential reimbursement consideration. We make no guarantee of reimbursement, as coverage decisions are made solely by the patient’s insurance carrier.

Surgery is rarely needed and is only appropriate for severe cases — significant disc displacement with locking, or advanced joint degeneration that has not responded to all conservative options. The vast majority of TMJ patients see meaningful improvement through neuromodulator therapy, night guards, and related conservative treatments.
